I think I have a bad lifter on my YJ (89, 4.2)
I have the valve cover off and know which cylinder is the problem, can anyone tell me if you have to pull the head on a 4.2 to remove a lifter?
My Haynes manual shows that you can on a 4 cylinder but doesn't mention it for the 6 cylinder.
Anyone ever done it????

It's a lot of work to change the lifters on a 4.2. How many miles on the motor? You can put new lifters on an old cam. You should determine if it is truely the lifter or if it is the cam. You'd need to test the lift using a dial indicator guage and rotating the crank and comparing the lift on all the cam lobes.
You will need to pull the head to get to the lifters if you think it is a lifter, and not a bearing. Does the noise go away after it warms up?

The lifter to the very front is the culprit, it is not moving at all, I can physically push down on the push rod side of the lifter and the lifter springs up and down easily
with some tension as if I was pushing down on a spring.
The other push rods do not have this kind of play in them, that is why I believe it to be the lifter.
The motor was completely rebuilt top to bottom and has less than 25,000 miles on it.
